LPN

  • Hourly wages will be determined after interview by case manager.
  • LPN will come to the office for scheduling or scheduling will be mailed via snail mail or e-mail.
  • Yearly evaluation.
  • Heartland will pay a portion each month as part of a cafeteria plan (vested after averaging 35 hours per week for 3 months) for your health insurance premium.
  • Heartland will pay mileage minus the first 15 miles from the office/home and minus the last 15 miles back to the office/home at $.29 per mile. Travel is a requirement for most homecare cases.
  • Heartland will make a matching 401K contribution at a rate to be determined when the employee becomes eligible.
  • An additional $.50 shift differential will be paid for shifts going past midnight and weekend shifts (any shift that includes Saturday or Sunday hours)
  • Staff must have a working phone and an alternative phone number for office personnel to easily reach the staff person.

RN
